In consideration of being permitted to participate in the volunteer activity(ies), the undersigned executes this Volunteer Waiver subject to the following terms:

The undersigned releases and forever discharges and holds harmless the LaPorte County Parks Department LaPorte County Park Foundation and the County of LaPorte as well as their members, directors, officers, agents, successors, and assigns (collectively referred to as the “County”) from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may arise as a result of the undersigned’s volunteer activity(ies).  The undersigned expressly and specifically assumes the risk of any and all harm that may or could occur as a result of the undersigned’s volunteer activity(ies) and carrying out or participating in such activity(ies).  This Volunteer Waiver is intended to be and shall be binding upon the devisees, heirs, executors, administrators, and personal representatives, if any, of the undersigned.
